New Patient Registration
If you would like to register with the Practice, please either attend the surgery to collect the registration form together with the new patient health questionnaire or download the forms using the links to print and complete at home. It is very important that you complete these forms fully. Please make sure you have your NHS number when completing the new registrations forms. Your NHS Number is a 10-digit number and it is unique to you. It helps healthcare staff and service providers identify you correctly and match your details to your health records.
You may be required to provide proof of address.
Before registering and completing forms, please ring the Receptionist on 01267 614111 to check if you are living within our practice area.
Please note: patients are registered with the practice, not an individual GP. However, you can at any time express a preference for a particular doctor and we will do our best to respect your choice. Please be aware, however, that not all doctors in the practice provide all services and specific doctors may not be immediately available.
Inform the receptionist if you have ever been a member of the Armed Forces when registering.

Please make every effort to ensure that your name, address and telephone/mobile numbers are kept correct.
As a Practice we are committed to providing a safe, comfortable environment for our patients. If you would like a chaperone during your consultation, please ask the doctor. A Practice Nurse or Health Care Assistant will usually act as your chaperone, but on rare occasions a Receptionist may be asked to chaperone.
You are able to see a doctor anywhere in the UK, if you are away from home and in need of medical help. You can do this by simply asking to see the nearest doctor, as a temporary resident. We will be happy to see any relatives or friends staying with you, if they need a doctor. If you are attending the Surgery for repeat medication that you have run out of or forgotten to bring with you, please note that selected Community Pharmacies in the area can provide you with an Emergency Supply.
